How Intranasal Drug Use Can Result in a Saddle Nose Deformity

Medicine nose, nasal spray or drops for health in white bottle, container in hand or palm from cold, flu, illness. Liquid pharmacy, care or treatment. Medical object, drug.

The nose is supported by a delicate framework of cartilage and bone. When that structural support is damaged, the bridge of the nose can collapse, creating a visible dip known as a saddle nose deformity. While trauma and certain medical conditions can cause this problem, intranasal drug use is one of the most well-documented causes.

At Saddle Nose Repair by Dr. Maurice Khosh, we treat patients from Manhattan and beyond who are seeking expert reconstruction after structural nasal damage.

How Intranasal Drugs Damage the Nose

Drugs that are inhaled through the nose come into direct contact with the nasal lining and septum. The septum is the central wall of cartilage and bone that separates the nostrils. It plays a critical role in supporting the bridge of the nose.

Substances such as cocaine are known to constrict blood vessels. This severe vasoconstriction reduces blood flow to the septal cartilage. Over time, decreased blood supply can cause tissue breakdown and cartilage necrosis.

Studies have shown that chronic intranasal cocaine use significantly increases the risk of septal perforation, where a hole develops in the septum. If the structural support continues to deteriorate, the nasal bridge may collapse, leading to a saddle nose deformity.

Although cocaine is one of the most common culprits, damage can also result from other intranasal substances. Certain medications that are inhaled through the nose, particularly when misused or overused, may contribute to tissue irritation and structural weakening. Chronic inflammation, infection, and repeated trauma further increase the risk.

What Is a Saddle Nose Deformity?

A saddle nose deformity is characterized by a noticeable depression along the nasal bridge. In more severe cases, the nose may appear shortened or flattened.

Beyond cosmetic concerns, patients may experience:

  • Nasal obstruction
  • Chronic congestion
  • Whistling sounds when breathing
  • Crusting or recurrent infections

The severity depends on how much cartilage has been lost. In advanced cases, significant structural reconstruction is required to restore both appearance and function.

Why This Type of Repair Is Challenging

Saddle nose repairs are among the more complex forms of rhinoplasty. When cartilage has been destroyed, simply reshaping the nose is not enough. The surgeon must rebuild the structural framework using grafts, often harvested from the patient’s own cartilage.

This requires advanced surgical skill and a deep understanding of nasal anatomy. Proper reconstruction restores support, improves breathing, and reestablishes natural contour.

Expert Repair by Dr. Maurice Khosh

Dr. Maurice Khosh is a respected facial plastic surgeon who specializes in saddle nose rhinoplasty and non-surgical repairs at his Manhattan clinic. Double board-certified in facial plastic surgery, Dr. Khosh brings years of experience and precision to complex nasal reconstruction.

He is recognized as an expert in treating all types of saddle nose deformities, including those caused by intranasal drug use. Using advanced techniques, Dr. Khosh carefully rebuilds the nasal structure to restore both function and aesthetic balance.

In select cases with milder deformities, non-surgical fillers may provide temporary improvement. However, moderate to severe cases typically require surgical reconstruction for lasting results.

Restoring Structure and Confidence

Intranasal drug-related damage can be physically and emotionally distressing. The visible collapse of the nasal bridge often affects self-confidence and breathing comfort.

At Saddle Nose Repair by Dr. Maurice Khosh, patients receive compassionate, expert care tailored to their specific condition. With advanced surgical techniques and over two decades of experience, Dr. Khosh helps restore both nasal support and facial harmony.

If you are experiencing structural nasal changes due to intranasal drug use or other causes, a consultation can provide clarity on your options for repair and long-term restoration.

Saw Dr. Khosh for a fractured nose. He reset my nose beautifully (and with little discomfort to me) and then discussed some of my sinus issues with me upon my follow up visit. Never felt like he was rushing me out and felt very confident in his abilities. I would highly recommend to anyone needing plastic or reconstructive options!

Upon hearing of the overwhelmingly positive experience a friend of mine had in choosing Dr. Khosh as her surgeon, I decided to make an appointment for my own septoplasty/rhinoplasty/turbinate reduction. Although I was slightly skeptical as the praise I had heard seemed a bit hyperbolic, all uncertainty dissipated after meeting the Doctor and his staff. My pre- and post-operative experience was excellent in terms of concerns adequately addressed prior to the surgery and mitigation of any bruising in the convalescent period. Despite my apparent predisposition to rather slower healing, a year after the surgery, my nose looks really pretty and my breathing entirely unobstructed! I highly recommend Dr. Khosh!

AMAZING!!!!!! I had Dr Khosh perform lipo on my chin/neck and my result is unbelievable! Better than I could have expected! I met with a couple of surgeons and the second I met with Dr Khosh I knew he was the one I was going with. He was compassionate and explained every detail of what would take place during the surgery and what to expect during recovery. I even emailed a few times during recovery with various questions and he got right back to me asap which was really comforting! I honestly fully recommend Dr Khosh, he did an amazing job and is an amazing person.
